藥理作用/作用機轉

 

適應症/

劑量

PO,

【Adult】

　●Genital herpes simplex:

　　* Initial episode: 400 mg TID for 7-10 days OR 200 mg 5 times daily (guideline dosing) for 10 days.

　　* Episodic therapy: 400 mg TID for 5 days OR 800 mg BID for 5 days OR 800 mg oTID for 2 days (guideline dosing); OR 200 mg q4h, 5 times daily for 5 days (manufacturer dose); initiate at earliest sign or symptom of recurrence.

　　* Suppressive therapy: 400 mg BID (guideline dose) for up to 12 months; alternative dosing, 200 mg orally 3-5 times daily (manufacturer dose).

　●Herpes zoster, Shingles: 800 mg q4h (5 times a day) for 7-10 days.

　●Varicella: 800 mg 4 times a day for 5 days

【Pediatric≧2 yr】

　　*≦40 kg: 20 mg/kg 4 times a day for 5 days.

　　* > 40 kg: 800 mg 4 times a day for 5 days.

【Geriatric】Refer to adult dosing.

『Renal Impairment: Adult』

CrCl 10-25：800 mg　Q8H

ClCr <10：200 mg　Q12H

『Hepatic Impairment: Adult』

No dosage adjustment necessary

使用禁忌

Hypersensitivity to acyclovir or valacyclovir.

Adverse Reactions (Significant): Acute kidney injury, Neurotoxicity, Thrombotic microangiopathy

【>10%】Malaise

【1% to 10%】 Diarrhea, nausea, vomiting 

【<1%】Anemia, leukocytosis, neutropenia, neutrophilia, thrombocythemia
